Hospice Social Worker
Lake Superior Hospice (LSH) is looking for a Medical Social Worker to fill a full-time position. The successful candidate will provide compassionate, caring, committed psychosocial evaluations and treatments to hospice patients in residential settings, participate in continuing education and improvement programs, and collaborate with team members. In addition, bereavement assessments and coordination of bereavement plans will be performed as needed. Following are some of the major responsibilities of this position:
- Evaluates social, emotional, financial and environmental factors related to patient’s illness and response to treatment
- Assesses special needs related to cultural diversity/developmental issues of patient/family
- Develops a social work treatment plan based on assessment and input from patient, family and other disciplines involved in care
- Collaborates with physicians, RNs and team members regarding significant social and emotional factors that affect the care plan
- Provides counseling related to end-of-life and grief issues and social work services to patients and their families including advance directives, funeral planning, powers of attorney, etc.
- Educates patients and/or family with information about community resources and assists with accessing resources as needed
- Continues to provide bereavement services to hospice families after the death of a loved one.
The Medical Social Worker is directly accountable to the Director of Clinical Services who reports to the CEO.
Position Qualifications
- Bachelor of Social Work degree with minimum of 5 years experience accepted.
- Master of Social Work degree from a school of social work accredited by the Council on Social Work Education preferred.
- Minimum of 1 year experience in a healthcare setting
- Valid driver’s license required
- Qualified candidates should possess the ability to work independently, as well as part of a team, caring for patients in their residence
- Required certification: current CPR certification
- Working conditions: may be exposed to patient elements; handles emergency or crisis situations; subject to irregular hours and occasional driving in changing weather conditions
